Diabetes Self-Management Education and Medical Nutrition Therapy
The USF Diabetes Center program is recognized by the American Association of Diabetes Educators as meeting the national standards for Diabetes Self-Management Education (DSME). Diabetes self-management education and Medical Nutrition Therapy (MNT) are offered at both individual appointments and group classes and are covered by most insurance companies and Medicare. The USF Diabetes Center is also a Certified Insulin Pump Training Center and offers education and training on all insulin pumps in the US marketplace.
Diabetes Self Management Education (DSME) classes are held for Children and Adults at the USF Diabetes Center. These classes are designed for those individuals who are newly diagnosed with diabetes, require a review of DSME skills, or are interested in or transitioning to insulin therapy. Classes are taught by Certified Diabetes Educators (CDEs) who are Advanced Practice Nurses, Registered Dietitians and Diabetes Specialist Physicians.

Type 2 Diabetes Group Classes
There are 3 ways to complete the curriculum. See the calendar below for the typical monthly class schedule.:
- One 8 - hour class: Held on the 1st and 3rd Wednesday of each month 8:30 AM - 4:30PM
- Two 4 - hour classes: Many offerings. Please see schedule.
*A follow-up session is scheduled for 3 months following the completion of the program. At this session, we will address your individual concerns, and help you to assess your specific diabetes goals.

Group Medical Nutrition Therapy Classes (individual appointments are also available) -
Includes diabetes nutrition counseling with a registered dietitian.
1st and 4th Monday of the month
2nd Tuesday of the month: 5:00PM - 7:00PM
5th Thursday of the month (if applicable)
2 hour session offered from 3pm - 5:00pm on the 3rd Wednesday and the 1st Thursday of the month.

How to Register
To attend our classes and have it covered by insurance or Medicare, we MUST have a Referral Form completed by your medical provider (MD, PA, or ARNP). You can obtain a Referral Form on this web site or by calling 813-396-2580. If you call, please have your provider’s name, phone and fax number available so we can fax this form to your provider’s office.
It is important for you to make sure in advance that your health insurance company recognizes the USF Diabetes Center, as a diabetes education provider for you. Insurance written in Florida must have a means of providing diabetes education to anyone holding that insurance, almost without exception.
Each person who registers for diabetes education class is encouraged to attend with a significant other. Please tell us who you will be bringing when you make your appointment. If you have a disability, a request for reasonable accommodations must be received 5 working days before the class
